Case Manager Position
The Palestinian American Community Center (PACC) is seeking immediate employment of a part-time case manager to lead our social service efforts for a minimum of 6 months. The Case Manager will be tasked with working closely with our Office Manager and all relevant PACC staff to guide each client that comes to us for support to the appropriate organization, government program, or any relevant social service relief platform that will help them achieve long-term and sustainable solutions, as well as immediate short-term relief if necessary. For example, PACC continues to be committed to addressing Food Insecurity as much as our resources allow and so the case manager would take the lead on disbursement. The Case Manager will also be expected to engage in continuous research of all existing and new resources so that we can constantly expand and be up to date in our networks.

General Responsibilities:
- Provide comprehensive case management services to clients, facilitating client advocacy, referral and service linkage via client outreach visits
- Promote becoming self-sustaining and provide long-term solutions to clients’ issues
- Provide client centered support services that are responsive to client and family’s needs
- Deliver culturally aware, sensitive and competent practice in client/family, community outreach, and advocacy
- Build and maintain ongoing collaborative relationships with community service providers, organizations and programs to coordinate care for each client
- Develop a comprehensive resource list that can be utilized by entire team
- Participate in meetings as expected/needed and be able to work well in a team and foster spaces of cooperative problem solving
- Complete all required documentation in a timely manner and meet all deadlines consistently
- If asked, represent PACC on social service committees, work groups, coalitions and other settings as assigned
